I'm sorry for your puppy! Unfortunately, no one will be qualified in these forums to answer you. This requires years of training as a nutrition consultant in veterinary medicine (DVM) 4 years plus 2-3 years of residence. For a consultation with a veterinary nutritionist, you can use the following services of the American College of Veterinary Nutrition Professionals (How to find a board certified nutritional consultant), Veterinarian:

Basically we are with you and your veterinarian about what the best combination for you and your dog may be too low phosphorus, low sodium and low% (but high bioavailability) of proteins. Good luck!
Who is collecting your favorite meat - especially as chicken - fried or steamed, and it - or you can use in the crock pot with half-cup of water do. Wait until they are soft enough to delete certain, then it is - you can use a few hooks - and then throw a handful of whole grain pasta or rice, and toss a handful of green beans. No onions! Furthermore, only try the chicken and mix with a cup or two of oats. You want moist but not wet and cool to room temperature, then it can be shaped into balls about an inch in diameter and more for them.
Green beans are for older dogs, fiber and helps arthritis good - my sister gave them to his 12 years, Rottweilers, and he has a great help.
You can actually do the same with almost any meat. But you want to stay not only for meat, since they have your fiber. It must be good, but makes it easy to use. You can just flat on your plate.
The good news is that the aid can be frozen for rice and nuked wiThout ruin the texture. You can pass raw meat through a food processor, but do not use the stop, and can be very hard to keep track.
